Obtaining a Certificate of Origin

1 - Completing an Exporter Information Form (C04)

  • Each company requiring export documentation is obliged to complete and return to Chamber of Commerce & Industry Queensland an Exporter Information Form (C04) to register the company and its authorised signatories.
  • Complete the Exporter Information Form (C04) and read about the fees and other information (PDF)
  • This form is only required to be completed once, however, Chamber of Commerce & Industry Queensland needs to be advised of any changes to company signatories or other company details.

2 - Preparing Certificates of Australian Origin

  • Original Certificates of Australian Origin (Form C06) are available to download here (PDF) or can be obtained for any of our offices. Information contained within the Certificate of Australian Origin must be consistent with details from a Bill of Lading/Airway Bill or Commercial Invoice.
  • Certificates of Australian Origin must be typed.
  • Certificates of Australian Origin must be signed and dated by authorised signatories of the company only after authorisation from Chamber of Commerce & Industry Queensland - refer to C04 - Exporter Information Form (PDF). If you are completing the Certificate of Origin template (PDF) you must sign and return the document to Chamber of Commerce & Industry Queensland.
  • To complete the Certificate of Origin template you will need to have Adobe Reader 7.0 or a later version installed. Adobe Reader is a free program used to view PDF documents. The latest version of Adobe Reader is available to download free from the Adobe website.

2a - Declaration of Origin

  • Declaration of Origin (PDF) forms must be typed, signed and dated by authorised signatories of the company prior to authorisation from Chamber of Commerce & Industry Queensland. Electronic signatures will not be accepted.

3 - Submission

  • The prepared Certificates of Australian Origin or Declarations of Origin must be preferably EMAILED to certification together with supporting documentation eg. Bill of Lading/Airway Bill of Commercial Invoice. These documents to be signed by the exporter AFTER have been received from CCIQ.
  • Exporters who deliver their documents by couriers or by mail MUST provide an additional copy/photocopy of the certificate, which will be retained, for Chamber of Commerce & Industry Queensland records.

Chamber of Commerce & Industry Queensland cannot certify certificates that have not been signed, contain incorrect or invalid information, or do not have supporting evidence (eg. Bill of Lading/Airway Bill or Commercial Invoice).

Any amendment to the Certificates following certification from Chamber of Commerce & Industry Queensland, must be returned to Chamber of Commerce & Industry Queensland for amendment verification.

Certificate of Free Sale & Certificate of Manufacture

Chamber of Commerce & Industry Queensland can issue and authorise a Certificate of Free Sale and a Certificate of Manufacture. To apply for either certificate, complete the templates listed below and return to Chamber of Commerce & Industry Queensland's International team for authorisation.
